What is post-concussive syndrome?

Not all concussions are created equal. Postconcussive syndrome occurs when symptoms persist after a concussion or mild traumatic brain injury. Unfortunately, brain health assessment is often overlooked or placed on a watch-and-wait plan, despite disruptions in specific brain areas that require reconnection. While most concussion symptoms resolve within two to four weeks, approximately 10-20% of individuals continue to experience symptoms, leading to silent suffering and dysfunction in daily activities. Clinical neuroscience or functional neurology is a methodical and integrative approach to treating disease and dysfunction of the nervous system. The brain and nervous system work together as a whole, not individually. Your brain is the master control. Everything that happens in your body is controlled by your brain. Every process that occurs in your body impacts the brain. The power of clinical neuroscience utilizes this bidirectional communication highway to create change. Neuroplasticity is the nervous system’s ability to reorganize itself by forming new connections between neurons throughout life. This is the mechanism by which functional neurology works. Neurological rehabilitation addresses eye movements, vestibular function, cognitive function, autonomic function, proprioception, reflex integration, balance and coordination function. The networks and specific areas of the brain are mapped out, and an individualized treatment plan is created.

Eye movements: Involved in every area of the brain to aid in vision and visual perception. They require proper position and movement to allow the eyes to focus and see the world.

Vestibular Function: The inner ear processes movement and communicates with the rest of the brain. The vestibular nuclei within the brainstem when balanced prevent dizziness and vertigo.

Cognitive Function: The ability to think and act is located within the frontal lobe. Imbalance can lead to brain fog, lack of endurance, anxiety and depression.

Autonomic Function: The autonomic nervous system is regulated by the hypothalamus and brainstem to allow mainly unconscious functions including heart rate, digestion, respiratory rate and pupillary response. Imbalance can lead to anxiety, gut issues, brain fog and fatigue.

Proprioception: Proprioception is the sense of movement and body position in space. This allows voluntary movements including posture, balance, and coordination.

Reflex Integration: Primitive reflexes allow survival and protection through early development and then these reflexes integrate. If reflexes are retained they impair proper brain development and growth leading to issues in cognition, emotional regulation and physical development.

Balance & Coordination:  A highly integrated system to know where the head and body is in space. Vision, vestibular and proprioceptive function all communicate to provide balance and coordination of thought and movement.

Dr. Prill received her certificate in craniosacral therapy to offer a gentle approach to releasing stress, trauma and adhesions within the brain and body. Craniosacral therapy seeks to restore the natural rhythmic movement found between the bones of the skull. It does the same for the movements of the sacrum. Memories and emotions are held within the fascia and cerebrospinal fluid, and this gentle therapy offers supportive ways to move through emotions. It also aids the circulation of the cerebrospinal fluid throughout the central nervous system. It often creates a relaxed and nourishing effect.

Laser and light therapy, also known as photobiomodulation and photoneuromodulation, is proven to be a safe and extremely effective modality to increase healing and recovery time for most ailments. If you are a cash-pay client coming in to utilize Dr. Prill’s services to recover from a concussion please book the two-part appointment package. This package includes the first two visits along with a road map to healing your brain. After the two initial appointments, you and Dr. Prill will decide if the neurologic rehabilitation package is right for you.

The brain uses neural networks involving the eyes, inner ear and muscles and joints to know where you are in space. These neural networks are often damaged individually or collectively causing confusion in the brain leading to many of the symptoms associated with concussions. Each concussion is unique to each individual based on multiple health factors including prior concussions, gender, systemic inflammation at the time of injury, menstrual cycle, effects on sleep, and stress levels to name a few.

Through a comprehensive intake and examination, Dr. Prill evaluates the visual system, the vestibular system, and the proprioceptive systems to see where there is dysfunction in the brain. Combining this information with your nutrition, lifestyle and overall health allows for an integrative treatment plan to allow the brain to heal and recover.

Through the comprehensive intake and examination, a nutrition, lifestyle and neurological treatment plan is created to meet your individual needs for brain recovery.

Neurological rehabilitation includes multiple types of stimuli to activate the brain and neural networks such as electrical modalities, laser and light therapies, auditory and visual inputs, and tactile and proprioceptive therapies. Dr. Prill is trained to find dysfunction in the brain, and then find ways to stimulate these areas to regain function. Neurological rehabilitation takes into account the fragile health of the brain to find balance and to build endurance. Using a series of exercises and sensory inputs, Dr. Prill begins to help the brain remember through integrating the regions of the brain and training them to work together again, while creating a safe environment for the brain to heal.

Treatment can last from 3 months to 1 year depending on severity of concussion.

At this time, Dr. Prill does not work with children, as her expertise is with adult brains. Child brain health is still in development and needs more specialized attention and consideration.

Regardless of fault, it is important to remember that you deserve treatment for your injuries.

When it comes to auto insurance claims related to injuries sustained in a motor vehicle accident (MVA), they are commonly known as Personal Injury Protection (PIP) claims. PIP coverage is typically included in car insurance policies, although it is possible to opt out for a lower premium. However, opting out of PIP coverage is strongly discouraged as it provides essential coverage for professional healthcare services.

Oregon Auto Insurance: What to Know

In Oregon, PIP coverage usually provides up to $15,000 for healthcare services, which must be utilized within 2 years of the accident. While this may seem like a substantial amount, it can be quickly depleted, especially considering expenses from hospital visits, emergency services, and multiple healthcare providers. Monitoring your PIP usage while receiving services is your responsibility to prevent unexpected out-of-pocket expenses. It is highly recommended to work with a lawyer who can guide you through this complex process effectively.

Following a car accident, it is crucial to seek legal representation. Attorneys typically work on a contingency basis, meaning they only receive payment once you settle your claim. Having a lawyer in place can provide invaluable guidance and support. Additionally, with legal representation, insurance adjusters are not allowed to contact you directly. Insurance adjusters work for the insurance company, and their main objective is to close your PIP claim as quickly and efficiently as possible.

Insurance companies often recommend Independent Medical Exams (IMEs) as part of the process to close your case. These exams involve evaluation by a doctor paid by the insurance company. It is important to be aware that these doctors may have a bias toward finding reasons to discontinue your care, as they primarily serve the interests of the insurance company. Even if you have remaining PIP coverage, an IME often signifies the end of PIP benefits. At that point, you may need to rely on your own private insurance or become a self-pay patient for further treatment. It’s important to note that our office does not bill any private insurance.

If your PIP coverage is exhausted, your lawyer may advise you to continue with care until you have fully recovered and are discharged. This may require a Doctor’s Lien, which is a signed agreement that defers payment to your healthcare provider until the claim is resolved through court proceedings. However, it’s important to note that Dr. Prill’s office does not accept Doctor’s Liens due to significant delays in receiving payment. In such cases, it becomes your responsibility to settle your bill and transition to a self-pay patient. You should discuss the financial aspects with your attorney, as reimbursement for these expenses may be included in your claim settlement.
